Prayer Wheel is a Rare Relic. Its effect causes all normal fights to drop two card rewards instead of one.

After ending a normal non-elite/boss fight, it will give two separate card rewards. Both can be activated, effectively doubling the cards the player receives from normal combat. This choice functions just like a normal choice after combat, and functions as expected with Question Card and Singing Bowl.

This relic does not spawn beyond floor 48, excluding Endless mode.

Old Prayer Wheel

Before Patch 25, Prayer Wheel was a common relic that made non-boss chests contain a card reward.
